By completing your Personal Questionnaire you will be relating your life story to your descendants.  You will tell them about your parents, grandparents, your childhood, your years as a teenager and as an adult.  You will write about your values, traditions, important events in your life, your talents, your children and your Jewish identity.

Download your Interview Questions to your word processing program, answer only the questions that you choose, and delete the others.  You may add your own questions if you like.  You may also print out the form and complete it in your own handwriting.
