Collect Your Family Photos...

By including photographs of your family in your file, you bring it to life for your grandchildren. Please have two copies made of your family photographs and include both copies with your submission. This will allow us to store a duplicate copy of your file in a separate location. Make sure that you write the names and birth dates (in pencil) on the back of each photo.
